Overview
Carpal tunnel syndrome supplement trials show clearer signals for symptom relief than for function. Alpha-Lipoic Acid may reduce pain in mild-to-moderate cases at 600 mg daily, though hand and wrist function often does not improve; IV-then-oral regimens may help more in some studies.
Topical Flaxseed Oil applied to the wrist alongside night splinting improved symptoms and function in small trials. Oral Vitamin B6 has not shown consistent benefit in higher-quality research despite early positive reports. Acetyl-L-Carnitine and Vitamin B2 (Riboflavin) evidence is limited or combination-based. - Taking vitamin B6 by mouth does not seem to help reduce symptoms of carpal tunnel syndrome.
- Most people with this condition are not low in vitamin B6, and studies show that supplements do not provide noticeable benefit.
- While some early research suggested improvement, later and better-quality studies have not supported this.
- A study in adults with mild to moderate carpal tunnel syndrome found that taking a mix of vitamin B6, alpha-lipoic acid, acetyl-L-carnitine, phosphatidylserine, turmeric, and vitamins C, E, B1, B2, and B12 twice a day for 60 days reduced pain and symptom severity but did not improve hand or wrist function.

- Getting alpha-lipoic acid by IV for 30 days, then by mouth for 60 days, may help more with pain and function than taking it by mouth alone.
- Some studies looked at alpha-lipoic acid combined with other ingredients.
- One study found that taking alpha-lipoic acid (600 mg) plus gamma-linolenic acid (360 mg) for 90 days improved symptoms and function, but the study did not compare it to a control group.
- Another study found that a mix of alpha-lipoic acid, acetyl-L-carnitine, phosphatidylserine, turmeric, and several vitamins (taken twice daily for 60 days) reduced pain and symptom severity, but did not improve hand or wrist function.
- It is not clear if the benefits were due to alpha-lipoic acid, other ingredients, or the combination.

- Small clinical studies have shown that applying flaxseed oil directly to the wrist, twice a day for four weeks, can improve symptoms and hand function in people with carpal tunnel syndrome who also wear a wrist splint at night.
- Symptoms improved by about 30%, hand function by 17%, and nerve functioning by 7%.
- In one study, flaxseed oil gel worked better than wearing a wrist splint alone.
- However, it is not yet known if these benefits last long term or how flaxseed oil compares with other common treatments.
