Overview

L-Carnitine (DEX Score 64; medium effect size, medium evidence, high trust) has medium-strength but mixed evidence for peripheral arterial disease. Taking 2 grams twice daily for three weeks may increase walking distance, but a single pre-exercise dose did not help, and adding L-carnitine to cilostazol showed no extra benefit. Cocoa (DEX Score 44; low effect size, low evidence, medium trust) flavanol-rich drinks improved six-minute walk distance over six months in one PAD trial.

Garlic (DEX Score 48; low effect size, medium evidence, medium trust) may modestly increase pain-free walking distance over 12 weeks. Alpha-Lipoic Acid (DEX Score 32; very low effect size, low evidence, medium trust) may reduce exercise pain but not exercise duration. See the heatmap and evidence reviews below.
