Overview
Multiple supplements show medium-strength evidence for metabolic syndrome across 84 studies. Vitamin B3 (Niacin) at about 2 g daily for four months may improve cholesterol and triglycerides but not post-meal blood sugar. Inositol may improve lipids, blood pressure, and insulin resistance over a year on a low-calorie diet.
Magnesium intake is linked with 27% to 31% lower metabolic syndrome risk in healthy adults. Berberine at 500 mg three times daily may reduce weight, blood pressure, and blood fats. L-Carnitine results are mixed across small trials. The heatmap and evidence reviews below provide a fuller comparison.

- In one very small study, intravenous L-carnitine (given twice daily for 7 days during a period of calorie restriction) led to greater weight loss, smaller waist size, and improvements in hunger, energy, and insulin levels.
- But another longer study found that taking L-carnitine by mouth (2 grams daily for 6 months) did not slow the growth of artery plaque and actually increased cholesterol and artery narrowing.
